Times when you MUST Use HTML tags:

There are times when the normal '=', '_', '*' shortcuts will not do their job: 

In Headers that will appear in a TOC (Table of Contents)

The above header is a perfect example -- the 'TOC' appears fixed font in the header itself, but as =TOC= in the TOC.  Or consider the HOSTNAME header I put in KnoppixHDInstall, Here are my attempts, in the order I tried them:

Look at them in the TOC above, here, & the raw text.  All look the same here, but radically different in the TOC. 

Somewhere at twiki.org there is a page where someone complains about having to use the HTML tags because the shortcut syntax won't work, but he is at a loss for examples.  Next time I'm there, I'll try to remember to post these.
